Hospital Pre-admission Procedures
Prior to hospital admission, please schedule a pre-admitting appointment by calling: 0049-69-6605-1199 or 1508 or 1133 or Fax : 0049-69-6605-1510 or 1530. The first pre-admitting appointments are at Monday afternoon regularly. You need a fixed scheduled time between 1:00 p.m. and 6.30 p.m.
The appointment will be at the ground floor of the
AMBULANZ. After entering the hospital you can ask at the front desk or you will go across the internal place of the hospital directly to the
AMBULANZ: There you will show your insurance card or your Champus claim form at the front desk. During the first appointment we will prove the indication for weight loss surgery. We will discuss, which procedures will be the best for you. All weight- loss-surgeries will be performed in laparoscopic techniques.
Please bring your:
- insurance cards,
- TRICARE patients: CHAMPUS CLAIM FORM
- copy of medical history and physical from the Physician that did your medical clearance
- a list of medications that you are currently taking with dosages and schedules, and, if desired, an advance directive/living will.
